Our Targeted Case Management (TCM) services are tailored to support individuals and families in Kentucky as they navigate complex medical, educational, and social landscapes. We provide comprehensive, no-cost assistance to those with a qualified mental health diagnosis who are currently in therapy.
Our case managers act as advocates and coordinators, facilitating access to necessary services, both Medicaid and non-Medicaid. We specialize in crafting personalized care plans, offering ongoing assessments, and ensuring seamless coordination among healthcare providers, legal advisors, and community resources.
Whether it’s dealing with family court matters, navigating CPS processes, or arranging educational and medical services, our dedicated team is here to support you every step of the way, ensuring no one has to manage these challenges alone.
TCM services are available at not cost to eligible individuals who:
Have a qualified mental health diagnosis
Are actively participating in therapy
Require additional support (detailed below in Comprehensive Case Management Services)
Our case managers provide a wide range of services tailored to meet the unique needs of each client:
Ongoing Comprehensive Assessment: Regular face-to-face assessments to determine the individual’s needs, involving the client, family, and other necessary collaterals.
Case Plan Development: Participation in crafting a detailed case plan that outlines the client's needs and the strategies to address them.
Service Coordination and Arrangement: Identifying and arranging the services required as outlined in the case plan.
Access to Services: Assisting clients and their families in accessing necessary services, navigating both Medicaid and non-Medicaid options provided by various agencies.
Monitoring and Progress Tracking: Continuous monitoring of the client’s progress through:
Making referrals
Tracking appointments
Removing barriers to service access
Performing follow-ups to ensure services meet the client’s needs
Periodic Re-assessments: Regular reviews of the client's needs to adjust the care plan as necessary.
Education on Early Intervention: Informing clients and families about the benefits of early intervention and treatment programs.
Advocacy: Active advocacy to ensure clients receive appropriate and timely services and interventions.
Record Maintenance: Keeping up-to-date client records that document all interactions, services needed, and progress.
Case Consultation: Providing consultation services to service providers to assist in evaluating the client’s progress.
Crisis Assistance: Offering immediate intervention and coordination of emergency services as needed.
Legal Navigation Support: Assisting clients in navigating legal processes related to dependency, neglect, custody, and other family court matters. Our team provides support in cases involving CPS (Child Protective Services), CHFS (Cabinet for Health and Family Services), and DCBS (Department for Community Based Services).
